Description:

The Department of Human Services (DHS), Ebensburg Center (EBC) is seeking a contractor to provide Medical Transport Service, non-emergency ambulance service to Ebensburg Center, 4501 Admiral Peary Highway, Ebensburg, PA 15931. EBC operates 24 hours per day 7 days per week.
The awarded Contractor must be licensed as an Advanced Life Support Service by the Pennsylvania Department of Health and enrolled as a Medicare and Pennsylvania Medical Assistance provider.
The anticipated term of the purchase order will be February 1, 2023, or date of full approval, whichever is later and end two years later with three options to renew for one year.
Quantities listed are estimates and may increase or decrease based on the needs of the facility.
Bids are due December 12, 2022, 1:00 PM and must be submitted electronically. Late bids will be disqualified. Contractors must enter firm prices for all contracted years and provide appropriate responses to all “Mandatory Attributes” contained in the solicitation. Failure to submit bid as described may result in yours being rejected as a non-responsive bidder.
DHS will award a purchase order to the lowest responsive and responsible contractor.

Contractors’ electronic submission must include the following as attachments. Failure to submit these items may result in the rejection of the Contractor’s submission:
1. Signed Lobbying Certification Form
2. A copy of a current license issued by the Pennsylvania Department of Health should be included in bid packet.
