Statement of Work – 6100058703
I. Objective:
The Department of Human Services (“DHS”) requires a Contractor to provide Catholic Spiritual Supports services to individuals at Ebensburg Center (“Facility”) located at 4501 Admiral Peary Hwy, Ebensburg, Pennsylvania 15931.
II. Term of Service:
The term of the Purchase Order will commence on the date it is fully executed and issued to the selected Contractor (“Effective Date”) and will expire three years after the Effective Date, unless it is terminated earlier pursuant to the terms of the Purchase Order. At its option and in its sole discretion, DHS may extend the term of the Purchase Order on the same terms and conditions for up to two additional one-year periods. At the time of each renewal the Department and selected Contractor may negotiate the unit prices not to exceed a maximum of a three percent (3%) increase over the unit prices in effect in the year immediately preceding each renewal term. The term of the Purchase Order may be extended by and at the sole option of the Commonwealth for up to 90 days upon the same terms and conditions where a continued need exists for the services of the selected Contractor and there has been no termination under the terms of the Purchase Order.??No work may begin or be reimbursed prior to Effective Date of the Purchase Order.? The selected Contractor will be paid after submitting invoices, provided such invoices are in accordance with the work plan and approved by?the Facility.
III. Scope of Work - Spiritual Support Services:
The selected Contractor shall be the direct provider of services at the Facility and shall provide the following service requirements.
A. Services. The selected Contractor shall provide Catholic spiritual supports services, including community inclusion spiritual support services to individuals at the Facility. All spiritual supports are to be provided in accordance with all pertinent federal, state, and local policies, procedures, and regulatory requirements, and in accordance with administrative and faith group direction.
B. Service Types. The selected Contractor may be required to provide the following types of services:
1. Faith group services of worship and religious rites
2. Visitation
3. Counseling
4. Memorial and funeral services
5. Ministries of prayer and blessing
6. Spiritual nurture, direction, and mentoring
7. Communication with families and community religious leaders
8. Interdisciplinary or multidisciplinary programs
9. Small group leadership geared towards development of spiritual and religious growth
10. Consultation with treatment or habilitative teams
11. Spiritual advocacy through faith group identity and interfaith meetings
12. Participation in interfaith services
13. Documentation of spiritual supports
14. Community inclusion spiritual supports (See C below)
15. Visitation to consumers from the Facility who have been hospitalized
16. Emergency spiritual supports for consumers experiencing sudden changes in their life such as the death of family members, physical illness, or emotional trauma
17. Other spiritual supports as determined by the needs of the consumers
18. All Chaplains shall complete Spiritual Assessments at the Facility
C. Community Inclusion Spiritual Supports. The selected contractor shall adhere to the community inclusion supports developed by the Facility and spiritual supports program. These supports may include:
1. Assisting those leaving the Facility in accessing community spiritual and religious supports according to each person’s choice of faith group;
2. Providing educational and training events for community spiritual leaders and congregations related to spiritual supports for consumers as they return to community settings from the Facility;
3. Developing and supporting programs to include consumers who live in the Facility in community congregations;
4. Developing and supporting programs of spiritual support from community congregations for consumers living in the Facility;
5. Other community inclusion spiritual supports as determined by the needs of the consumers served.
D. Reporting and Communication.
The selected Contractor shall be responsible for submitting the following to the Program Director:
1. Completion of timesheets as directed by Contract Monitor (“CM”)
2. Completion of narrative work summaries as directed
The selected Contractor shall be responsible for contacting the CM in accordance with Facility expectations when unable to provide scheduled services (call-off procedure)
E. In the absence of a Spiritual Supports Services Director (“SSSD”) at the Facility or at the discretion of a SSSD, the selected Contractor may serve as a spiritual supports services representative on committees such as Human Rights and Ethics Committees.
