Description:

The Department of Human Services (DHS), Warren State Hospital is seeking a contractor to provide Non-Denominational Protestant Chaplaincy Spiritual Supports services for individuals located at DHS Warren State Hospital, 33 Main Drive, Warren PA based on the attached Statement of Work.
The quantities are estimated only and may increase or decrease dependent upon the needs of the department.
Bids must be submitted electronically. Late bids will be disqualified. Contractors must enter firm prices for all contracted years and provide appropriate responses to all “Mandatory Attributes” contained in the solicitation. Failure to submit bid as described may result in yours being rejected as a non-responsive bidder.
Bidders will provide their cost by entering their per unit cost; a unit is described as EA which is equal to 1 Hour of Spiritual Support Services for the designated unit on each line for each year of the Purchase Order. Units are defined in the Statement of Work and will include ALL attenant costs including travel. Travel should be calculated in accordance with the Commonwealth Management Directive 230.10.
Award will be made to the lowest responsive and responsible bidder.
