The Department of Human Services (DHS), Office of Children, Youth and Families, Bureau of Juvenile Justice Services (BJJS), Loysville Complex is seeking a contractor to provide Oral Surgery/Restorative Dental Services to adjudicated juveniles (consumers) residing at the Loysville Youth Development Center (LYDC), 8 Opportunity Drive, Loysville, PA 17047 in Perry County.
Quantities listed are estimates and may increase or decrease based on the needs of the facility.
Bids are due 2/17/23, 1:00PM and must be submitted electronically. Late bids will be disqualified.
Contractors must enter firm prices for all contracted years and provide appropriate responses to all “Mandatory Attributes” contained in the solicitation. Failure to submit bid as described may result in yours being rejected as a non-responsive bidder.

Contractors’ electronic submission must include the following as attachments. Failure to submit these items may result in the rejection of the Contractor’s submission:

Signed Signature Page that will be made part of the attached draft Agreement.
Bidders must have a current license with the State of Pennsylvania and be a Board Certified or Board Eligible Oral Surgeon. Documentation of current license and certification or eligibility must be submitted with bid.
ONLY ELECTRONIC BID RESPONSES WILL BE ACCEPTED FOR SUBMISSION. All bidders are responsible for monitoring the DGS emarketplace website to view any change notices to this IFB. The change notices shall become incorporated as part of this bid. In order to submit your Bid Response, interested vendors must be registered. Award will be made based on lowest and responsible and responsive bidder and will be based off of line item price basis and the completed MA Rate Certification form indicating the optional percentage.
All references to a “Purchase Order” in the terms and conditions or within the Solicitation documentation shall be substituted with either the term Contract, or the phrase “formal written notice to proceed” as appropriate or vice versa.
The contractor is to provide Oral Surgery Services at Medical Assistance (Medicaid) rates plus or minus an optional percentage of facility gross charges. The fee schedule can be found at:

The contractor is required to submit the attached MA Rate Acceptance Certification, which also identifies the optional percentage rate you will be requesting if you choose to add a percentage.
For the MA Rate line: Estimated Service Cost of $20,000.00. For purposes of the electronic submission, all responders should bid $1.00 each as the unit price on the bid, so that the total cost of the estimated service for Line 1 for MA rates will then be $20,000.00.
Invoice prices will be based off the MA Rate Certification Form filled out.
Contractor will provide services at usual and customary charges for services that do not fall under the MA Rates. Please see attached Common Services Rate Sheet. The contractor will also complete the pricing on the Common Services Rate Sheet with signature and submit with bid. The contractor will additionally provide a Price List of their usual and customary charges of all other services not listed which will remain in effect for the entire term of the contract.
